Description
Basic Qualifications:
The Physical Therapist must be licensed in the state of practice and be free from any disciplinary action. The PT is expected to have a valid driver’s license and good driving record.
Other Qualifications:
The position requires clear understanding of therapy in the homecare setting. Ability to assess the needs of the clients and work with individuals within and outside the company to meet the needs of the client is required. The position requires demonstration of strong analytical, problem solving, communication, and leadership skills. Computer proficiency and ability learn different computer programs are highly required.
Physical Requirements
Lift, push, and/or pull minimum 50 lbs
Extended periods of walking, standing, and driving required
Significant potential for exposure to Latex
TB testing per regulations

Responsibilities:
Evaluates and implements physical therapy program on referral of client’s physician. Provides written evaluation and plan of care, Writes progress notes and discharge summaries according to departmental policy.
Coordinates physical therapy with nursing staff and other rehabilitative services.
May provide in-services to administration, medical staff, other therapists and family members.
Attends meetings as needed at home health agency and attends mandatory in-services.
Attends required educational in-services to meet license renewal requirements.
Supervision of PTA and delegation of treatment plan.
Will employ and follow all appropriate agency infection control policies and procedures.
Observes, records, and reports to the physician the client’s reaction to treatment and changes in the client’s condition.
