POSITION DESCRIPTION
We are looking for a licensed Physical Therapist to support the Advanced Physical Therapy and Recovery Team. This person will use appropriate tests and measures, clinical judgement, and evidence-based practice to properly screen, evaluate, and diagnose patients with a range of orthopedic conditions. The therapist will provide clear communication and effective treatment as part of the physical therapy plan of care.

Primary Responsibilities
- Exhibits behaviors and communication skills that demonstrate SHIFT’s commitment to superior customer service, which includes delivering superior service and empathetic care in every member interaction
- Delivers high-quality physical therapy services to members, including assessment, treatment, program planning and implementation, related documentation, and communication
- Communicates effectively with physicians, SHIFT care team members (medical and extended), and related service vendors, including external care providers, hospitals, and patients, among others
- Uses, protects, and discloses SHIFT patients’ protected health information (PHI) in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards.
- Develops a plan of care with an appropriate duration and frequency based on anticipated goals, predicted level of improvement, and specific treatment interventions
- Creates fitness, physical therapy and wellness-oriented programs tailored to members’ specific needs

Requirements
Qualifications
- Strong organizational and time management skills
- Excellent written and verbal communication skills.
- Certifications in dry needling, blood flow restriction, and/or other manual therapy, preferred
- Comfortable working within all Microsoft Suite products, Adobe Acrobat, and WebPT
- Works well in a team, bringing and encouraging positive energy
- Diligent, resilient, positive, articulate, and professional
- A passion for integrated personal health and wellness
- 1-3 years of experience in a dynamic, demanding environment
- Doctorate degree from an accredited physical therapy program
- Current physical therapy license in State of Illinois
- Current CPR certification
